The goal of Medical Homes for All of Arizona's CSHCN is to improve the quality of pediatric health care and related services needs. This project builds upon the existing infrastructure of the ADHS/OCSHCN Parent Leadership teams to develop a network of medical homes that include parents, primary care physicians, specialty care providers and care coordinators.

Parents are critical to the identification of medical care needs unique to their communities and providing consumer input regarding the quality and direction of their health care network. Parents are the articulate consumers for their children.

Each of the Medical Home teams identified in this brochure insert are in the process of or have selected a physician(s) and/or medical practice(s) that participates collaboratively in determining what changes are needed to become a Medical Home using the seven quality indicators.
