A Medical Home is not a building, house, or hospital.
It is an approach to providing health care services in a high-quality
and cost-effective manner. Children with special health care needs
receive the care they need from physicians they trust. The professionals
and families work together to identify and access additional medical
and related services needed to help children achieve their full
potential. A medical home has the following features:
| Accessible: |
Care is provided in the child's community. All
insurance, including Medicaid, is accepted. |
| Family-Centered: |
Family is recognized as the principal caregiver
and center of support for children. |
| Continuous: |
Pediatric health professionals provide care from
infancy through adolescence.
Health care is available 24 hours, 7 days a week. |
| Comprehensive: |
Assistance with transitions to home, school,
and adult services is provided. Preventative, primary and tertiary
care are addressed. |
| Coordinated: |
Families are linked to support, educational and
community-based services. All records and information are kept
in this central source. |
| Compassionate: |
Concern for the well-being of child and family
is expressed and demonstrated. |
| Culturally Effective: |
The family's values, beliefs and behaviors are
respected.
