Racing to the Future: Transition of Arizona Youth with Special Health Care Needs

Racing to the Future is dedicated to establishing a community system of comprehensive youth-centered services that enable youth with special health care needs (YSHN) to make effective transitions to all aspects of adult life.

Three approaches define our comprehensive goal: a) YSHN engage in Participatory Action Research (PAR) to pursue personal and group goals and use technology to monitor health status and communicate with others, b) parents of YSHN engage in PAR to build their capacity to assist their youth and to deal with their concerns about the youth's transition to adulthood, and c) a network of pediatricians and adult providers create the infrastructure necessary for physician collaboration in the care of YSHN.
